To provide personal care to the terminally ill Hospice patient as needed. To provide assistance and understanding to the family in the home care situation and in time of bereavement. To work as a member of the Hospice team in providing Hospice care.
The primary function of the Hospice Aide is the personal care of the patient.
Adequately understand, follow, and complete assignments. Obtain vital signs, record, and notify Director of Nursing of deviations as instructed.
Assist the patient in activities of daily living and help re-establish the activities of daily living.
Bathing the patient or assisting to bathe and assisting with skin care.
Assisting with transfer activities in and out of bed to a chair or wheelchair.
Assisting to ambulate - may include assistance with walker or crutches.
Assist to bathroom, commode chair, or bedpan.
Assisting patient to dress and / or undress.
Help keep a safe environment in areas of the home by the patient.
Perform such incidental household services essential to the patients’ health care as assigned by the RN.
Report all pertinent information to the Director of Nursing.
Other duties as assigned or required by the Director of Nursing.
Educational/Degree: High school graduate preferred. Must have completed at least the tenth grade and be reasonably efﬁcient in spelling and writing.
Knowledge/Skills/Ability: Must be literate, emotionally mature adult with desirable personality and character traits, e.g., pleasant, courteous, willingness to carry to assignments and follow instructions. Must have a sympathetic attitude towards the ill and elderly.
Experience: At least 1 year of experience in home health care, a nursing home or hospital is preferred.
